A risk score combines signals — a delivery address that differs from billing, a new account, an unusual value, a card issued elsewhere — into a number, and the number is only useful if somebody has decided what happens at each level.

The design that works has three outcomes rather than two: accept, refuse, and review. Collapsing it to accept or refuse forces every borderline order into one of the two expensive errors, whereas a review queue costs a few minutes and recovers orders that were never fraudulent. Which brings up the measurement nobody does. The orders you refused are invisible afterwards, so the false positive rate is unknown unless you deliberately sample them and check. Without that, a rule set drifts towards caution over years because every fraud loss is visible and every lost genuine customer is not. The other rule we apply is that the reason for a decline is never shown in detail, since a precise explanation is also instructions for getting past it next time.
