Category URLs, and what breaks when you reorganise
A URL that contains the whole category path looks tidy and welds every product to a position in a structure you will eventually want to change.
A flat product URL is uglier and survives reorganisation completely, because the product does not move when the category above it does. That trade is the whole decision, and the flat version wins more often than people expect — catalogues get restructured every few years, and the tidy path is only tidy until the first time a product belongs in two places. What matters more than either choice is what happens during the change. Every address that had accumulated any visibility needs to keep resolving, which means a redirect written at the moment the structure changes rather than reconstructed afterwards from a crawl. The failure we see most often is not a wrong decision about URL shape; it is a reorganisation carried out over a weekend by somebody who did not know that the old addresses were worth anything, and a quarter spent wondering where the traffic went.

Should the category be in the product URL?
Usually not. It welds the product to a position in a structure you will want to change, and it stops making sense the first time something belongs in two categories. A flat product address is less tidy and survives every reorganisation you will do over the store's life.
What has to happen when the structure changes?
Every old address has to keep resolving to its equivalent, written as part of the change rather than reconstructed later. The map is worth more than the reorganisation itself, and rebuilding it afterwards from a crawl recovers some of the addresses and never all of them.
Is it ever worth changing URLs on an existing store?
Sometimes, when the current structure is actively confusing or blocks something you need. It is never worth doing casually, because the cost is paid immediately in traffic and the benefit arrives slowly. If nobody can state what improves, the answer is to leave them alone.
